USCIS I-693 Immigration Medical Exam

As a USCIS-designated Civil Surgeon, Elevated Primary Care is authorized to perform the required medical examination for applicants seeking adjustment of status (Green Card) in the United States, as documented on Form I-693, Report of Immigration Medical Examination and Vaccination Record.

The 3-Part I-693 Medical Examination Process

The examination is a comprehensive review as required by the U.S. Citizenship and Immigration Services (USCIS) and includes:

1. Screening and Physical Examination:

• A review of your complete medical and immunization history.
• A thorough physical examination.
• Mandatory screening for communicable diseases of public health significance, including:
• Tuberculosis (TB)
• Syphilis
• Gonorrhea
• Leprosy
• Screening for significant physical or mental disorders with associated harmful behavior, as well as drug abuse and addiction.

1. Vaccination Review and Administration:

• Review of your current vaccination records to ensure compliance with USCIS requirements.
• Required vaccines typically include: MMR, Varicella, Tdap, Hepatitis B, Influenza, and COVID-19 (as per current USCIS requirements). Additional vaccines may be required based on age and medical history, particularly for those under 18 or over 65.
• Vaccines are available on-site, but the cost is separate from the examination fee. We will advise you on how to best utilize your insurance or other resources for vaccine coverage.

1. Form I-693 Completion and Sealing:

• The 14-page Form I-693 will be accurately completed, signed, and sealed by our Civil Surgeon, confirming all required medical and vaccination criteria are met.
• You will receive the completed form in a sealed envelope, which must remain unopened for submission to USCIS.
